Andover Surgery Center, LP -Insurance / Billing
INSURANCE & BILLING

Insurance and Billing
Before Your Surgery – A representative of the Center will contact you to obtain insurance information to simplify the registration process. If you will not be available by telephone, call the Center at 978-475-2880 between 8AM and 5PM.

It is your responsibility to determine, in advance, the extent of your insurance coverage. You may need the assistance of your insurer, your employer or your doctor.

Prior approvals or second opinions required by your insurance must be obtained in advance of surgery.

The Day of Surgery – Be sure to bring your insurance identification card, as the receptionist will need to verify all insurance information.

Payment – We will bill your primary insurance company but you may be responsible for any deductible, coinsurance or balance not paid by insurance. After surgery, we will bill you for any unpaid balance that must be paid within 30 days.

If you do not have insurance coverage, or if you are having cosmetic surgery, you will be required to pay the Center on admission. Our business office will provide you with an estimate prior to surgery and a final statement after the procedure is completed.

Your surgeon will bill you separately. If you have used anesthesia or pathology services during your procedure, you will be billed separately.

You may pay by MasterCard or Visa.

Work-Related Surgery
If your surgery is the result of an accident at work, we will require specific information before surgery including the name of your employer’s workman compensation carrier, the agent or person to contact at work, the complete address of your employer at the time of injury, the date and time of your injury, the time of your accident and your claim number.
